Navigating the New Model of Care (MoC) Pathways (Hospitals)

(Hospitals) Care delivery across Saudi Arabia is being redesigned around a patient-centric national Model of Care encompassing 42 clinical interventions across six pathways: Keep Well, Planned Care, Safe Birth, Chronic Care, Urgent Care, and Last Phase. This program equips clinical directors, department heads, and nursing supervisors to translate the framework into daily ward operations. While familiarity with isolated pathways such as Safe Birth is high, systemic integration across facilities remains fragmented. Participants learn to dismantle departmental silos, break inter-facility communication barriers, and design referral protocols that move complex patients fluidly through the continuum of care, from rural primary health centers to urban medical cities, without redundancy, delays, or handoff gaps. The course uses national reference frameworks and lessons from forums such as the Saudi Model of Care Forum to build operational blueprints. By the end, leaders can map their service lines onto the six pathways, redesign multidisciplinary handoffs, and embed continuity of care as a measurable institutional standard aligned with Vision 2030 and the Health Sector Transformation Program.
